Georgia Medicaid's SOURCE program provides Service options Using Resources in a Community. This program is for qualifying individuals 65 years of age or older with chronic medical conditions, or individuals under 65 years of age and disabled.  Individuals eligible for enrollment must be fully covered by Medicaid and meet other eligibility requirements.  For more information visit:  https://georgia.gov/apply-service-options-using-resources-community-environment-source

Georgia Medicaid's Community Care Services (CCSP) Program is a waiver program that provides in-home and community-based services for people who are elderly, disabled, or functionally impaired.  For more information on CCSP please visit:  https://dch.georgia.gov/sites/dch.georgia.gov/files/related_files/document/17CCSP.pdf

Meals on Wheels is a program that delivers hot meals to homebound elderly utilizing volunteers.  Menus are prepared by a licensed dietician and follow the guidelines set forth by the state of Georgia. Located at 1500 2nd Ave Columbus GA 31901.  For more information call:  706-327-5208

River Valley Area Agency on Aging connects older adults and persons with disabilities to local resources and services to meet their needs.  Visit:  https://rivervalleyaging.org/

The Georgia Department of Human Services Division of Aging Services assists older individuals, other at-risk adults and their families and caregivers to live safe, self-reliant lives.  For more information visit: https://aging.georgia.gov/

The Georgia Aging and Disability Resource Connection works to build on existing networks to create Aging and Disability Resource Connections (ADRC) in communities by providing information, assistance, and access to those in need of long-term support.
